Position Overview

Ultimate Staffing Services is actively seeking a dedicated and knowledgeable Community Based Program Specialist to join the team in San Jose, California. This position plays a crucial role in developing, supporting, and monitoring a network of public and community-based providers and vendors. The role focuses on delivering Enhanced Care Management (ECM), Community Supports (CS), and Community Health Worker (CHW) services, as well as addressing social determinants of health.

Responsibilities

  • Develop and maintain updated knowledge of community-based services and capacity across Santa Clara County, supporting the identification and assessment of ECM, CS, and/or CHW provider network gaps.
  • Identify initial and ongoing needs to support ECM, CS, and/or CHW delivery systems and operational infrastructure.
  • Oversee the development and management of the ECM, CS, and/or CHW provider network, serving as a knowledge and resource expert.
  • Establish and manage positive working relationships with all providers and vendors delivering services under ECM, CS, and/or CHW.
  • Receive, research, and respond to provider inquiries and issues in a timely manner.
  • Conduct orientation and ongoing training for community service providers and office staff.
  • Support the Department Manager in generating reports and performing special projects.
  • Troubleshoot with providers to address claims and encounter data submission issues for CHW services.
  • Understand and track applicable regulatory and reporting requirements, ensuring accuracy and compliance.
  • Attend off-site meetings or events as necessary and perform other related duties as assigned.

Qualifications

  • Bachelor's degree in public health, social welfare, or related field; or equivalent experience, training, or coursework.
  • Minimum of two years of direct work experience related to the responsibilities described.
  • Demonstrated experience leading/managing projects and initiatives.
  • Knowledgeable in home and community-based services and community resource networks.
  • Deep understanding of social determinants of health impacting Santa Clara County.
  • Advanced proficiency with Microsoft Suite applications and applicable software.
  • Excellent oral and written communication skills.
  • Ability to facilitate meetings and deliver presentations effectively.
  • Valid California driver's license and acceptable driving record.
  • Knowledge of community-based delivery systems and managed care is desired.
  • Project Management Professional (PMP) certification is desired.

Required Work Hours

Monday - Friday, First Shift

Benefits

Max Pay is $44. Min Pay is $35.
